I would like to place an order for OMNICYCLES, Version 2.0, "Lifelines".   I have already downloaded and installed the shareware version on my system. I understand and agree to the following:

1) I understand that when I order OMNICYCLES, I am purchasing a registration number which will activate the program's  most special feature: the ability to prepare predictive graphs for the future, for the person (or persons) for whom I am purchasing that right, or else (in the case of the "unlimited" version) the ability to do future graphs for anyone. The program will, once I enter the registration number that OMNICYCLES will send me upon ordering, generate graphs for all future dates, with no time limitation, and for all the different themes contained in the OMNICYCLES Version 2.0 "Lifelines" program.

2) My purchase includes FREE updates for any newer versions which you may prepare during the first TWO YEARS after the date of purchase.

3) In addition, I will be entitled to FREE technical support by e-mail (Omnicycles@omnicycles.com).

4) When ordering the program, I will give you the birth date(s) for the person(s) for whom I am purchasing the ability to do future graphs. Then, when you at Omnicycles.com have received both this order form as well as the purchase price, you will send me my "registration number" which, when entered into the program, will activate the "future predictive graphs" feature for the person or persons whose birth dates I have provided. In the case of the "unlimited" version, entering the registration number will allow me to do future graphs for everyone.

Note: The birth dates are necessary for us to generate your code number. We will, of course, treat this information as confidential, and will not give it to any third party! (Note: If you purchase the "unlimited" version, there is no need to give us the birth dates of the individuals, since this version allows you to do future graphs for as many people as you wish.)
5) Due to the nature of the product (software), my purchase will be considered final.

The NAME, however, is not necessary; if you don't wish to give the names, simply type "A" for person 1, "B" for person 2, and "C" for person 3 in the NAME fields.
